Comitancillo lies in the department of San Marcos, in the western highlands of Guatemala. The municipality covers mountainous terrain, and much of its population lives in villages and hamlets spread across the slopes rather than in a single dense town. On a map, roads curve with the land, following ridges and valleys.
The municipal centre forms a small, compact core, with a church, a central square and streets laid out close around it. Many residents belong to the Mam Maya community, and the area's life is closely connected to farming on the surrounding hillsides.
