Itambacuri lies in northeastern Minas Gerais, in the hilly country of the Mucuri region. The landscape here is made up of rounded hills and narrow valleys, and the town fits into it. Streets gather on the lower ground and climb the slopes where they can, so the street pattern looks compact and irregular.
The town grew from a nineteenth-century mission settlement founded by Capuchin friars, and its centre is still arranged around a main church and square. From there, streets extend along the valley and up the hillsides. Main roads leave town toward the other centres of the region, including Teófilo Otoni.
