Montecatini-Terme lies in the Valdinievole, the valley of the Nievole in the province of Pistoia, between Florence and Lucca. The lower town is one of Italy's best-known spa resorts, laid out around a large park that holds the thermal establishments. Broad avenues, hotels and villas spread out in a fairly regular pattern, and to the north the ground rises sharply toward the old hilltop village.
That village, Montecatini Alto, is a compact medieval settlement reached from the valley by a funicular that has run since the late nineteenth century. The thermal buildings below, including the Tettuccio with its colonnades, drew visitors from across Europe. In 2021 UNESCO inscribed the town as part of the Great Spa Towns of Europe.

On the hill to the north, the medieval village forms a small, dense cluster of lanes around a central piazza. It is clearly distinct from the spacious town below.
