Altinho lies in the agreste of Pernambuco, the transitional belt between the humid coast and the dry sertão. From above the town is compact: a few dozen blocks around a central core, with roads leaving in several directions across the countryside. The settled area is modest, so the pattern of the surrounding land takes up much of the frame.
The municipality is mostly rural, and the map reflects that. Around the town, farm tracks and small roads cross gently rolling ground, connecting outlying communities to the centre. As in many interior towns of the Northeast, the central square and main church anchor the street grid, and ordinary life still runs along a few main streets.
