Cayey lies in the mountainous centre of Puerto Rico, in a high valley surrounded by the ridges of the Sierra de Cayey. Its cooler, often misty climate has earned it the nickname Ciudad de las Brumas, the city of mists. On a map, the town's compact grid around the central plaza contrasts sharply with the curving mountain roads that surround it.
Highway 52, the main route between San Juan and Ponce, passes by the town, drawing a long, confident line through the hills. Elsewhere, smaller roads twist along ridges and valleys to rural barrios. One of these, in Guavate, is widely known for its roadside restaurants serving slow-roasted pork, a popular weekend destination for people across the island.
