São José do Egito lies in the Sertão of Pernambuco, in the valley of the Pajeú River, one of the main watercourses of the semi-arid interior. The river runs low or dry for much of the year, but on the map its course still shapes the town, with streets laid out near its banks and land rising gently into caatinga-covered hills around them.
The centre follows the pattern of many interior towns in north-eastern Brazil: a main church and square, commercial streets leading off them and residential blocks extending in a loose grid. Roads leave for neighbouring towns of the Pajeú region, and beyond the last houses the map becomes sparse, with farm tracks, small reservoirs and scattered settlements.
