Komatsushimachō lies on the eastern coast of Tokushima Prefecture, just south of Tokushima city, facing the Kii Channel. From above the coastline is the dominant feature: a series of harbour basins, breakwaters and straight-edged reclaimed land that give the shore an engineered, angular outline quite unlike the natural curves of the coast further south.
Inland the town becomes a mixture of residential streets, rivers and small wooded hills rising from the plain.
