Nakanojō covers a large, mostly mountainous area in the northwest of Gunma Prefecture. On a map the settled land is narrow: the town centre and its neighbourhoods follow the valley of the Agatsuma River and the smaller streams that feed it, while wooded hills rise on every side. Roads and a railway line keep to the valley floor, so the street network reads as a long, winding band.
Within the town's boundaries lies Shima Onsen, a hot spring village strung along a narrow river valley to the north and known for its old wooden inns.
