Middelburg lies on the high, open grasslands of Mpumalanga, east of Pretoria, and its map reflects the flat terrain. The older town centre follows a regular grid of streets, while newer suburbs spread outwards in curving crescents. Railway lines and the N4 highway pass close by, cutting long, straight lines across the landscape and linking the town to Gauteng in one direction and the Lowveld in the other.
The town is closely tied to the surrounding coal fields and to steel production, and large industrial sites sit on its outskirts as broad blocks with few streets. A river runs past the town and a dam provides a patch of open water nearby, which relieves the otherwise dry pattern of roads and plots.
