Middletown occupies the centre of Aquidneck Island, between Newport to the south and Portsmouth to the north. On a map, the island's shape does most of the work. Water lies on both sides, Narragansett Bay to the west and the Sakonnet River to the east, and the town's roads run lengthwise down the island between them.
The southern edge meets the Atlantic. There the shoreline breaks into beaches and rocky points, including Second Beach and Sachuest Point, which is protected as a national wildlife refuge. Inland, residential streets, farmland and commercial corridors mix in an uneven pattern that becomes quieter as it approaches the coast.
