Esfarāyen lies in North Khorasan Province in northeastern Iran, in a valley bordered by mountains. On a map the town appears as a compact grid of streets surrounded by cultivated land, with roads leading out across the valley toward neighbouring settlements and the provincial capital, Bojnurd.
The area has been settled for a very long time. Near the town stand the remains of Belqeys Citadel, a large mud brick fortress whose outline is still visible and which is a registered national heritage site.
