From above, Karlovy Vary is mostly valley. The Teplá river twists north through a deep cleft in the hills, and the old spa quarter lines both banks in a thin, continuous ribbon. Streets can't spread sideways, so they climb in switchbacks or run along the water, and the forest reaches almost down to the colonnades.
At the northern end, the Teplá flows into the wider Ohře, and the map opens out. Newer districts, bridges and a looser street grid sit on the flatter ground around the confluence. The contrast is easy to read in black and white: a tight, sinuous spa core to the south and a broader town spreading along the Ohře to the north.
The town grew up around its hot springs, which still feed the drinking colonnades along the river, and each summer it hosts one of Europe's oldest international film festivals. To the north the Teplá joins the Ohře, and the tight valley gives way to wider streets, bridges and the flatter ground where the modern town spreads out.

Dense woodland covers the slopes on either side of the spa quarter, leaving large blank areas that frame the thin band of buildings along the river.
