Kotō lies in Kumamoto Prefecture on the island of Kyushu, on the broad plain around the city of Kumamoto. From above, it has the typical texture of a Japanese residential district: narrow streets, irregular blocks and small plots packed closely together, with a few wider roads carrying traffic across the plain. In the wider landscape, rivers flow west from the Aso highlands toward the Ariake Sea.
The region is shaped by Mount Aso to the east, one of the largest active volcanoes in Japan, whose caldera and grasslands feed the rivers and groundwater of the plain. Kumamoto is known for the quality and abundance of its spring water, and for Kumamoto Castle, which has been under restoration since the earthquakes of April 2016.
