Aksay lies on the Don River, immediately east of Rostov-on-Don, and its map is shaped by the river and the steep bank above it. The older part of town runs along the water at the foot of the slope, while newer neighbourhoods spread across the flat land at the top. Ravines cut down toward the river, and the streets bend around them rather than crossing directly.

One of the most prominent features is the bridge of the M-4 Don highway, which crosses the river at Aksay and carries much of the traffic between Moscow and the south of Russia. The town has a long history as a trading and customs point on the Don, and a historic customs outpost building from the eighteenth century has been preserved as part of a local museum.
