Douako lies in the Kankan Region of Upper Guinea, an area of wide savanna, open woodland and farmland drained by the upper tributaries of the Niger River. The landscape is gently rolling, with seasonal streams and patches of trees between cultivated fields, and settlements linked by a network of roads and footpaths.
The town has a modest built-up area, with compounds and small streets gathered around its center and thinning out quickly into the countryside. The pattern is loose and practical, shaped by the paths that lead to fields, water and neighbouring villages rather than by any formal plan.
