Itaperuna lies in the northwest of Rio de Janeiro state, far from the coast, in a region of hills and farmland. The Muriaé River runs through the town, and it is the defining line on the map: the centre sits close to its banks, and bridges tie the neighbourhoods on either side together.
The streets follow a fairly regular pattern near the centre, becoming looser toward the outskirts, where roads climb onto low hills or run out toward the surrounding farms. Development follows the easier ground, so the town's outline stretches along the river rather than forming a neat circle.
