Ottaviano lies in the province of Naples, on the northeastern flank of Vesuvius. The town stretches from the plain up toward the slopes of the volcano, and its map reflects this. Streets on the lower ground are denser and more regular, while roads higher up follow the contours and thin out as they approach the woods and lava fields of Vesuvius National Park.
The historic centre is overlooked by the Palazzo Mediceo, a large palace once held by a branch of the Medici family, which stands on a rise above the town. Around it, older streets form an irregular pattern that contrasts with the straighter roads of later growth. The land around is intensively cultivated, with vineyards and orchards on fertile volcanic soil.
Ottaviano has been shaped repeatedly by the volcano, including the 1906 eruption, which hit this side of the mountain especially hard.
