Central Islip sits near the middle of Long Island in the Town of Islip, Suffolk County, on the flat ground typical of the island's interior. Its map is a patchwork of suburban street grids and curving subdivisions, cut through by the Long Island Rail Road, which stops here on the Ronkonkoma Branch, and by the wide roads that link it to the rest of the island.
The most distinctive part of the plan is the land once occupied by the Central Islip State Hospital, a large psychiatric institution that operated for about a century.
