San Andrés Itzapa lies in the department of Chimaltenango, in the central highlands of Guatemala, a short distance from the departmental capital. Like many towns founded in the colonial period, it was laid out on a grid around a central plaza and church. The town is widely known for its shrine to San Simón, also called Maximón, a folk saint venerated across the highlands.
