Castilho lies in the far west of São Paulo state, on the Paraná River where it forms the boundary with Mato Grosso do Sul. The town is laid out in the straight, regular grid typical of settlements in this part of Brazil, and on the map it sits close to a very large body of water: the river here is broad, widened by the reservoir of the Jupiá dam.
The surrounding land is flat to gently rolling interior country, used for farming and cattle, and crossed by long, direct roads. Across the river lies Três Lagoas.
