Payathonzu lies in Kayin State directly on the border with Thailand, at the place long known as the Three Pagodas Pass. The town's name refers to those three small pagodas, which stand near the frontier line. On a map, the settlement is modest and irregular, its streets following the valley floor between forested ridges rather than any planned grid.
The pass has been a route between the Burmese and Thai lowlands for centuries, used by traders and, historically, by armies moving between the two kingdoms. In the Second World War the Thailand–Burma Railway was built through this area, and the pass is still associated with that line. Today a road crosses the frontier here toward Sangkhla Buri on the Thai side.
