Tierralta lies in the department of Córdoba in northern Colombia, on the Sinú River where the Caribbean lowlands begin to rise toward the mountains. The town sits beside the river, its streets forming a regular grid, while to the south the land climbs into the forested ranges of the Paramillo massif.
Upstream from the town is the Urrá dam and its reservoir, which holds back the waters of the Sinú, and further south lies Paramillo National Natural Park, one of the country's large protected areas. On a regional map the contrast is striking: a compact town on one side, and wide wooded terrain and open water on the other.
