Itahari sits on the level plains of the eastern Terai in Sunsari District, Koshi Province. The city owes its growth to its location: here the Mahendra (East-West) Highway crosses the north-south road linking Biratnagar, near the Indian border, with Dharan at the foot of the hills. From above, that junction is clearly the centre, and the city's streets spread out from it in every direction.
Around the crossroads, the city has expanded quickly into what was once farmland. The map shows dense commercial strips along the highways and newer residential blocks among rice fields and ponds. The land is almost entirely flat, and the streets, though not a strict grid, have the regular, open quality typical of Terai towns.
