Iizuka lies in the Chikuho region of inland Fukuoka Prefecture, in a basin ringed by forested hills. The Onga River runs north through the city, and the densest streets gather along its banks. Railway lines and main roads follow the valley floor. Away from the centre, housing spreads into side valleys and up the lower slopes, while the ridgelines stay largely empty.
From the late nineteenth century to the mid twentieth, Chikuho was one of Japan's most important coalfields. Iizuka grew as the commercial centre for the mining towns around it. The Kaho Gekijo, a traditional wooden kabuki theatre from that period, still stands in the city and is still used for performances. Earlier, Iizuka was a post town on the old Nagasaki Kaido road.
