Karthikappally lies in Alappuzha district on Kerala's low coastal plain, a region shaped by water. The Arabian Sea is close to the west, and backwaters, canals and lagoons crisscross the land behind the coast. On a map, the settlement is less a compact town than a spread of houses and lanes among coconut groves, paddy fields and waterways.
Roads here often run alongside canals or on the raised strips between fields, so the street pattern follows the water. The main coastal highway runs through the area and gives the map a long north–south line. On either side, smaller lanes and channels make a fine, irregular mesh that is typical of the Kuttanad region's edges.
