Malacatán lies in the department of San Marcos in western Guatemala, close to the border with Mexico. It sits on the lower Pacific slope, where the highlands of the Sierra Madre drop toward the warm coastal plain. On a map, the town appears as a fairly regular block pattern in the middle of open agricultural land, with roads leading off toward the border and toward the rest of the department.
The surrounding landscape is shaped by water running downhill toward the Pacific, and streams and river corridors cross the countryside around the town.
