Makumbako lies in the Southern Highlands of Tanzania, in Njombe Region, on high, open ground. It is primarily a crossroads. The main highway between Dar es Salaam and Zambia passes through the town, and here a road branches south towards Njombe. The TAZARA railway also runs through the town and has a station there.

These routes shape the map. The commercial centre lies along and near the road junction, with markets, depots and transport yards, while residential areas spread out on either side in a loose pattern. Beyond that, the upland landscape is mostly farmland and grassland, with plantations in the wider region.
