Cachoeiras de Macacu lies in the interior of Rio de Janeiro state, where the lowlands around Guanabara Bay meet the steep mountains of the Serra do Mar. The name refers to the waterfalls of the Macacu River, which comes down from the highlands and runs through the municipality toward the bay.
On a map, the town centre sits in the valley, with streets following the river and the main road. Above it, the terrain rises quickly into forest. Much of the upland area is protected, including part of the Três Picos State Park, and the map becomes a maze of contour-hugging roads and streams.
