Ottapalam lies in the Palakkad district of central Kerala, on the banks of the Bharathapuzha, the state's second-longest river. The river dominates the map: a broad, braided band of water and sandbanks sweeping past the town. The town's roads and railway line run roughly parallel to it before branching inland.

The Bharathapuzha valley holds a special place in Kerala's culture. The poet Kunchan Nambiar was born in the area, and many classical art traditions developed along the river. More recently, the green, quiet landscape around Ottapalam has become a familiar setting in Malayalam cinema, with its riverbanks and old houses appearing in many films.
