Kaltenkirchen lies in Schleswig-Holstein, about 30 kilometres north of Hamburg, in the flat, open landscape of Holstein. The town has grown strongly as part of the Hamburg metropolitan area. On a map, its centre sits along a main road, surrounded by residential districts, business parks and green belts.

Two transport lines define it. The A7 motorway runs past on the western side, heading north towards Denmark and south to Hamburg, with its own junction for the town. The AKN railway line gives Kaltenkirchen direct commuter trains to the city, and its route cuts a long line through the settlement. Fields and woodland lie beyond the edges.
