Ometepe rises from Lake Nicaragua, also called Cocibolca, as two volcanic cones linked by a low, narrow neck of land. Concepción, the northwestern cone, is active and almost perfectly symmetrical; Maderas, to the southeast, is older, forested and topped by a crater lake. From above, the island has a distinctive figure-eight outline that makes it one of the most recognisable shapes in Central America.
Roads loop around each volcano near the shore, connecting towns and villages such as Moyogalpa, the main ferry port on the west side, and Altagracia on the northeast coast. Ometepe is widely described as the largest volcanic island in a freshwater lake, and it is known for its pre-Columbian petroglyphs and for statues carved in basalt.

The southeastern cone is broader and more rugged, with fewer roads and scattered villages on its lower slopes. On the map it balances Concepción with a softer, less regular outline.
